You’re about to begin the CFTC Relief Grant Nomination Process

The Care for the Caregivers (CFTC) Relief Grant is reserved for live-in primary carepartners (i.e. spouses, offspring) caring for their loved one with FTD or a diagnosed dementia. Nominations must be from care professionals that can vouch for the care partner and LOWD’s situation. Example: Doctor, hospice or social worker involved in the care of your loved one with dementia.Support group facilitator from a professional organization such as Alzheimer’s San Diego, Lorenzo’s House, Association of FTD (AFTD), etc. All nominators will be verified.
